Begin by navigating to the Physical Exam Addr/Phone page.

Use the Physical Exam Addr/Phone page to enter the examiner's address and phone data and the date and type of the physical exam.Step 7
Use the Exam Date field to enter the date when the exam was administered. The default is the system's current date. You can override this date if needed. In this example, retain the default date.Step 8
Select the type of exam. In this example, it is an annual exam.

Notice that the Next Exam field defaults to one year from the date specified in the Exam Date field based on selecting the exam type of Annual.Step 11
If the ID number of the physician who administered the exam exists in the database, specify it in the Physician ID field. When you specify the physician ID, the name of the physician appears automatically. If the ID number does not exist, you can specify the name of the physician or the specialist who administered the exam in the Physician field.

There are three check boxes on this page:
• Baseline: When selected, indicates that this exam is the baseline for tracking physical condition over time.
• Organ Donor: When selected, indicates this individual is or wants to be an organ donor.
• Blood Donor: When selected, indicates this individual is or wants to be blood donor.
In this example, the student donates blood on a regular basis.

Use the Referral field to enter the place or people to which this individual is referred (Optometrist, Employee Assistance Program, Hospital, and so on) for further examination or treatment.Step 22

You have successfully entered physical exam information for an individual.
