Entering Immunization and Health Information


Concept


Steps:

  1. Begin by navigating to the Immunization page.

     

    Click the Campus Community link.

  2. Click the Personal Information link.

  3. Click the Immunizations and Health link.

  4. Enter the desired information into the ID field. Enter

     a valid value
     e.g.
     "0058"

  5. Click the Search button.

  6. Use the Immunization page to enter immunization data for an individual.

  7. Use the Immunization field to enter the immunization code, as defined in the Immunization Table, that this individual has received.

     

    In this example, the student received an immunization for polio.

     

    Enter the desired information into the Immunization field. Enter

     a valid value
     e.g.
     "POLIO"

  8. The Immunization Number field defines the number of this immunization on the list of immunizations for this individual.

     

    The system automatically enters the next sequential number for each immunization that you add. You can override the number to reorder the list of immunizations.

     

    In this example, retain the default of 1, as this is the first and only immunization the student received.

  9. Use the Date Taken field to enter the date that the individual received this immunization.

     

    Click in the Date Taken field.

  10. Enter the desired information into the Date Taken field. Enter

     a valid value
     e.g.
     "06/15/1984"

  11. If needed, use the Expiration Date field to enter the date when the immunity expires.

     

    In this example, leave the field blank.

  12. Use the Date Received field to enter the date when you received, or are entering, this health test information.

     

    In this example, leave the field blank.

  13. Use the Immunization Status field to define the current status (Complete, Unknown, or Partial) of this immunization.

     

    Click the Immunization Number: list.

  15. When you save the page, the Immunization Criteria section of the page is populated with the criteria or test data that is associated with this immunization code.

     

    Click the Save button.

  16. The criteria for this immunization is to take a blood test for Hepatitis A to verify if the individual is immune.

     

    In this example, the student received the immunization, so the test does not apply.

     

    Click the Does Not Apply option.

  17. Next, record the student's health test information.

     

    Click the Health Test tab.

  18. Use the Health Test page to enter or review data from an individual's health test.

  19. Use the Health Test field to enter the health test code, as defined in the Health Test Table, that this individual has taken.

     

    In this example, the student had a tuberculosis test.

     

    Enter the desired information into the Health Test field. Enter

     a valid value
     e.g.
     "TB"

  20. The Health Test Number field defines the number of this health test on the list of health tests for this individual.

  24. If the test has a specific result that can be quantified, use the Test Value field to enter the test result value expressed in the alphabetic, numeric, percentage, or other form by the medical community. For example, AB or O (blood types) or 177 (cholesterol).

  25. Use the Test Result field to enter the overall test result (Negative, Neutral, or Positive). Some tests, like screening for cancer, would have a negative or positive result. Other tests are neutral.

     

    Click the Test Result list.
