Entering Accommodation Data


Concept


Steps:

  1. Begin by navigating to the Accommodation Request page.

     

    Click the Campus Community link.

  4. Enter the desired information into the ID field. Enter

     a valid value
     e.g.
     "0058"

  6. Use the Accommodation Request page to enter and track an individual's accommodation request.

  7. The Accommodation ID field indicates the number of this request on the list of this individual's accommodation requests.

     

    The system displays the next sequential number for each accommodation request that you add. You can override the number to reorder the list of accommodation requests.

     

    In this example, this is the first request for this individual, so you will use the default.

  8. Click in the Date of Request field.

  10. Use the Comment field to enter comments that further identify the accommodation request for this individual.

  12. Use the Responsible ID field to enter the ID of the individual at your institution who is managing this request.

  14. Select the Pending, Accepted, or Undue Hardship option to indicate whether the request is pending , evaluated and accepted, or evaluated and denied due to an undue hardship on the department or institution.

     

    In this example, you will retain the default of Pending.

  15. In the Request Status Date field, enter the date that the request was changed to a status of pending, accepted, or denied.

     

    In this example, retain the default date.

  16. Use the Regulatory Region field to enter the code for the country whose regulations apply to this request.

     

    Click in the Regulatory Region field.

  17. Enter the desired information into the Regulatory Region field. Enter

     a valid value

  18. Use the Diagnosis Code field to enter the that describes the illness or disability for which accommodations have been requested.

     

    Click in the Diagnosis Code field.

  22. In the Option ID field, the system displays the number of this option request in the list of options for this accommodation request. The system displays the next sequential number for each option that you add. You can override the number to reorder the list of options. For this example, retain the default value.

     

    Select the Employer Suggested Option check box to indicate that a staff person at your institution suggested this option.

     

    Click the Employer Suggested Option option.

  23. In the Currency Code field, enter the currency in which the cost is expressed.

  30. Use the Accommodation Status and Status Date fields to enter the status of this option, (such as accepted, approved, consider, offered, or rejected), and the date of the status.

     

    In this example, retain the default status of Consider and use 01/15/2007 for the status date.

You have successfully created an accommodation request. 

 

To summarize, use the Accommodation Data component to create accommodation requests for individuals having certain health restrictions and disabilities. You can create requests for several types of accommodations, such as purchasing special equipment or making structural changes to a classroom or a work environment.
