Steps:
Begin by navigating to the Eye Exam Addr/Phone page.
Click the Campus Community link.
Step 1Click the Personal Information link.
Step 2Click the Health Information link.
Step 3Click the Eye Exam link.
Step 4Enter the desired information into the ID field. Enter
Click the Search button.
Step 6Use the Eye Exam Addr/Phone page to enter the examiner's address and phone data, and the date and type of eye exam administered.
Step 7Use the Exam Date field to enter the date of the exam. It defaults to the current system date, but can be changed if needed. For this example, use the default date.
Step 8Select the type of exam. In this example, it is an annual exam.
Click the Exam Type list.
Step 9Click the Annual list item.
Step 10Notice that the Next Exam field defaults to one year from the date specified in the Exam Date field based on selecting the exam type of Annual.
Step 11If the ID number of the physician who administered the exam exists in the database, specify it in the Physician ID field. When you specify the physician ID, the name of the physician appears automatically. If the ID number does not exist, you can specify the name of the physician or the specialist who administered the exam in the Physician field.
Click in the Physician ID field.
Step 12In this example, the ID for the physician is in the database.

Click in the Phone field.

Click the Exam Details tab.
Step 17Use the Exam Details page to enter eye exam details, including results, referrals, and comments.
Step 18In the Far Sight and Near Sight regions, you specify the individual's corrected and uncorrected vision. The Corrected field specifies the individual's visual sharpness with corrective lenses. In the Uncorrected field, you specify the visual sharpness level without corrective lenses.
In this example, John does not wear corrective eyewear and his vision is 20/20 in both eyes.

You should select the Baseline check box if you are using the result of this exam to track visual sharpness over time. For this example, you do not need to select this check box because you are not using the result of the current exam as a baseline.
Step 35If you select the Correction Required check box, it indicates that the individual requires lenses. For this example, John does not need corrective lenses because the eye exam result shows that his visual sharpness is normal.
Step 36The Color Vision group box contains two option buttons. The Normal option is the default. This indicates that the individual has normal color perception. If the individual does not perceive colors properly, select the Abnormal option.
For this example, retain the default selection for color vision.
Step 37The Referral field indicates whether the individual is supposed to visit another doctor or hospital. Use the Comments field to add further details about the individual's eye exam.
Step 38Click the Save button.
Step 39You have successfully recorded the details and results of an eye exam for an individual.
To summarize, use the Eye Exam component to record and track details of a visual acuity exam, such as corrected vision, color vision, and the need for corrective lenses.
